Chronic Diseases

We are part of the HSE chronic disease management programme which includes the following conditions:

  • Asthma

  • Chronic Obstructive Airways Disease

  • Ischaemic heart disease/Heart attack

  • Atrial Fibrillation

  • Heart failure

  • We also offer the General Practice management of all other chronic diseases which are not part of this programme.
